SAMPLE ANSWER
Dear Chief,
I write this letter to inform you that I am seven months pregnant. I am currently receiving medical treatment and my doctor has given 25 November 2015 as the due date for delivery.
I have low vitamins and feel lethargic. I am not comfortable to look after myself at
this point of time. Ergo, instead of 8 weeks leave starting from 08TH November 2015, I need 4 weeks extra to cope with my health condition. Additionally, I want, to inform you that I have my full annual leave allowance of four weeks plus one month of maternity leave. I have attached the medical certificate issued by Scarborough Beach Medical Hospital describing my health issues for your perusal.
I wish to join office sooner if circumstances change in my favour. If it is so, I will notify you well in advance. I request you to approve my leave at the earliest.
